Does the Sigtonics S-58 come with a push to talk switch as part of the headset?

No, you will have to order a PTT separately as the manufacture does not know if you are using a portable or panel mount intercom.

Is this a standard aviation model? We are looking for a headset to plug into Icom A14 Air band portable. Also must have PTT switch. What do you suggest?

Yes, this is the standard dual GA plugs. You will need adapter part number 11-18537 and PTT switch part number 11-18529.
