No, there are no forms included in the box. They can be furnished with 8130-3 forms, but it must be added at time of order. Additional charges will apply.

Yes, this is approved via FAA TSO.

Do we have to buy cables separately to plug it?

Per the manufacturer, this is only the turn coordinator. The cables are sold separately.
